How to treat high light chain 9 Months, 1 Week ago

My brother have high light chain and high B2M. All other test results are normal or near normal. He is on hemodialysis due to renal failure. He was on thalidomide initially, then Velcade. Now he is on his 7 th cycle of Velcade, but the numbers are still high. My question is what we should do next, any other therapy that you recommend to treat the light-chain type. thanks.

B2M will be high in anyone on dialysis since that protein is cleared by the kidney and not effectively in dialysis. Lots of other options for people that involve combinations of drugs if Velcade or Thal alone do not work.

Could you recommend any specific treatment options other than velcade and the combinations with velcade for my brother. velcade and combinations are not effective to him, and he is light-chain type of MM with kidney failure. Thank you very much.

what is the dose to start taking Revlimid, 25 mg or 5 mg, and then, decrease or increase based on reactions? Is renal functionality a factor for Revilimd dose or not. Thanks
